Cigarette, medical oxygen cause fatal Tulalip fire

MARYSVILLE, Wash. (AP) — The fire that killed a woman on the Tulalip Indian Reservation near Marysville started when she lit a cigarette while breathing medical oxygen from a tank.

Firefighters were unable to rescue 58-year-old Phyllis R. Ballam from the flames Wednesday night.

The Snohomish County medical examiner’s office says she died of smoke inhalation.
