5 reprimands for Oregon State Hospital death

SALEM, Ore. (AP) — Oregon State Hospital has reprimanded five employees for inadequate care of a patient who died in October at the Salem mental hospital.

The Statesman Journal reports an investigation in the death of Moises Perez found all five failed to perform their duties. Two of the five are nursing leaders and three are veteran staffers.

The 42-year-old Perez was found dead Oct. 17 in his bed. An autopsy found he died of heart disease.

State Addictions and Mental Health Division Director Richard Harris says the reprimands are appropriate sanctions.

Employees could have been fired or had their pay docked.

The letter of reprimand stays on an employee’s record for three years.
