The Health Professions Quality Assurance Office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In September, the Nursing Assistant Program charged certified nursing assistant Erica L. Vazquez with unprofessional conduct. She was convicted of theft.

In September, the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Thomas Allen Breitenbauch and placed his certification on probation for two years, and set conditions against his credential.
