The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In April, the Veterinary Board amended the statement of charges on veterinarian Michael L. Baker, who is alleged to have committed unprofessional conduct and may defend against these charges.

In April, the secretary of health indefinitely suspended the credential of nursing assistant Derrick Bohrer. While working at an assisted-living facility, Bohrer was charged with diverting patient medication, and tested positive during a drug screening for controlled substances.

In April, the Nursing Commission ended probation for registered nurse Sylvia Lynn Kearns.

In April, the Chemical Dependency Professional Program granted a chemical dependency professional trainee credential to Aaron John Larson and placed him on probation for at least three years. Larson was convicted of second-degree theft in 2011.

In March, the dental commission withdrew charges against dentist and dentist moderate sedation with parenteral agents permit holder Sanjeev K. Sharma.

In April, the Oregon State Board of Nursing revoked the Registered Nurse license of Leona M. Smith, of Vancouver, for failing to respect client rights and dignity, violating client rights to privacy, failing to answer questions truthfully, and failing to cooperate with the board during the course of an investigation.
