Goodwill fined $9,000 for fatal Lynnwood accident

EVERETT — The state Department of Labor and Industries has fined Goodwill $9,000 for workplace safety violations in a forklift accident that killed a man in August in Lynnwood.

The department says Goodwill should have done a better job training the forklift operator.

The Daily Herald reports the state agency released it findings Wednesday.

The operator’s view was obstructed. He didn’t see a man in front of the forklift until too late.

When the forklift stopped suddenly in the parking lot, a load of merchandise fell off and killed 69-year-old Chuck K. Lee of Bothell.
