The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In November, the Home Care Aide Program denied home care aide-provisional and home care aide credentials to Jeffery Bryon Koupal who, in September, was denied a registered nursing assistant credential.

In December, the Nursing Assistant Program charged registered nursing assistant Samantha Meredith Horner with unprofessional conduct. The Department of Social and Health Services found that Horner had physically abused a vulnerable adult. Horner was placed on the Residential Care Services Registry, restricting her ability to practice as a nursing assistant.

In December, the Nursing Assistant Program agreed to reinstate the registered nursing assistant credential of Mayra I. Garcia and placed her credential on probation for at least one year.
