The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In November, the secretary of health indefinitely suspended the credential of paramedic Sarah Roskam. She relapsed and was diagnosed with opioid dependence after completing a substance abuse program.

In November, the Nursing Care Quality Assurance Commission suspended the credential of licensed practical nurse Paula Ann Harris for at least three years. Harris was deemed unable to practice with reasonable skill and safety. She was hospitalized in a psychiatric unit.

In November, the Nursing Care Quality Assurance Commission suspended the credential of registered nurse Leona M. Smith for 18 months. Smith’s credential was revoked by the Oregon State Board of Nursing for taking and texting a photo of a patient’s bare buttocks without their knowledge or consent.
