The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In January, the Pharmacy Commission charged pharmacist Merlin L. Fish with unprofessional conduct. Fish allegedly didn’t comply with terms of probation, didn’t reimburse the commission for $1,000 in costs, and didn’t provide proof of completing course work in pharmacy law and ethics.

In January, the Nursing Assistant Program charged certified nursing assistant David Michael Zinn with unprofessional conduct. Zinn was convicted of two felonies and three gross misdemeanors between 2013 and 2015.
