The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary action or has withdrawn charges against these Clark County health care providers.

In May, the secretary of health ended probation for chemical dependency professional trainee Henry Paul Dzur.

In May, the Unlicensed Practice Program entered an agreement with Dwight Karlsen that requires him to cease and desist from practicing massage without a license, and to pay a $250 fine. Karlsen, who doesn’t have a massage practitioner credential, advertised offering massage therapy.
