The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In August, the Nursing Commission indefinitely suspended the licensed practical nurse credential of Margaret Lynn Doyle. Doyle allegedly tested positive for alcohol after reporting to work smelling of alcohol and slurring her speech.

In September, the secretary of health ended conditions on the ambulatory surgical facility credential of Salmon Creek Plastic Surgery P.C.

In October, the Oregon State Board of Nursing suspended the practical nurse license of Kristy M. McCarter for a minimum of 14 days for failing to cooperate with the board during the course of an investigation. She resides in Vancouver.

In October, the Oregon State Board of Nursing placed the registered nurse license of Trudy E. Urban on probation for 24 months for using intoxicants to the extent injurious to herself or others, and practicing nursing while impaired. She lives in Battle Ground.
