Health Worker Report

The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In August the Dental Commission charged dentist Phuong-Oanh Thuy Tran with unprofessional conduct. Charges say Tran submitted Medicaid claims for services Medicaid didn’t cover, and received at least $1 million to which Tran wasn’t entitled. Tran allegedly submitted improper billing for an extraction. Tran’s treatment of one patient allegedly required the patient to seek subsequent care. Tran allegedly offered another patient a treatment option that wasn’t called for. In October, the Dental Commission indefinitely suspended Tran’s dentist credential.

In August, the Cardiovascular Invasive Specialist Program agreed to indefinitely suspend the cardiovascular invasive specialist credential of Kirt Hughes. Hughes diverted a syringe of fentanyl meant for a patient and substituted another syringe that didn’t contain fentanyl.

In August, the Nursing Commission charged registered nurse Deanna Marie Hancock with unprofessional conduct and immediately suspended her credential. In 2019 Hancock surrendered her Oregon registered nurse license.

In August, the Unlicensed Practice Program notified Luis Munoz of its intent to issue a cease-and-desist order. Munoz allegedly lacks a chiropractor license, but advertised and performed “alphabiotics, the alignment process” for a fee.

In September, the Nursing Assistant Program charged certified nursing assistant Keri Kaylene Picone with unprofessional conduct and immediately suspended her credential. The state Department of Social and Health Services, which found that Picone financially exploited a vulnerable adult, placed her on its long-term care abuse and neglect registry. That prohibits her from being employed in caring for or having unsupervised access to vulnerable adults.

In October, The Oregon State Board of Nursing accepted the voluntary surrender of Eric L. Burke’s registered nurse license for the unauthorized removal of drugs from the workplace and entering inaccurate documentation into a health record.

In October, the Dental Commission charged dentist Constant E. Lu with unprofessional conduct. Lu allegedly provided inadequate or needless treatment to multiple patients.
