State honors Area Agency on Aging and Disabilities of Southwest Washington

Hazel Dell — The Washington State Council on Aging gave its 2019 Excellence in Action Organization Award to the Area Agency on Aging and Disabilities of Southwest Washington. The Vancouver-based agency earned the award in collaboration with Aging and Disability Services King County, Southeast Washington Aging and Long-Term Care Council of Governments, and Northwest Geriatrics Workforce Enhancement Center. Together, the organizations worked to create a primary care liaison within the regional area agencies on aging who works to engage primary care clinics, including health care providers and other members of the health care team, in order to raise awareness of aging network programs and encourage use of those resources. The liaison also provides stable support to build and sustain cross-system relationships.
