Vancouver Clinic and Humana partner again

Vancouver Clinic and Humana, a health insurance company, will continue their partnership at Vancouver Clinic’s newest location in Vancouver.

The Evergreen Place clinic, 709 N.E. 136th Ave., Vancouver, is set to open Friday. It will be operated by Vancouver Clinic and will accept all Humana Medicare Advantage HMO and PPO plans offered in Washington, as well as a Humana Dual Eligible Special Needs Plan for certain patients qualifying for both Medicare and Medicaid, according to a Vancouver Clinic press release.

Evergreen is modeled after the Vancouver Plaza clinic, which has the same Humana partnership. Both clinics are accepting new patients.

The neighborhood clinic model seeks to give patients more time with providers, has a focus on preventing illness and improved care for people with chronic conditions. The clinics are generally for older populations or those who need a higher level of support.

“We’ve seen positive patient outcomes from the first neighborhood clinic and its unique model of care,” Vancouver Clinic CEO Mark Mantei said in a statement. “The neighborhood clinics offer integrated care teams that include doctors, nurses, health coaches, nutritionists, medication management, and social services resources to help guide patients through the complex health system and ensure their care addresses the whole person.”
