SEATTLE (AP) — The Washington Health Department says deficiencies at Seattle Children’s Hospital contributed to the death of a baby. The state agency has directed the hospital to make corrections.

A state report released Tuesday said the baby being prepared for transfer to the hospital in September was given medication by a nurse without a doctor’s order. In a review, the department found a total of 12 cases in which the hospital failed to ensure there were signed orders from doctors for drugs.

In a response, the hospital said it strongly disagreed about the cause of the baby’s death. The hospital says the King County medical examiner determined the baby died of natural causes.

The hospital says it cooperated with the investigation and will comply with the state’s requirements.
