The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In April, the Nursing Commission charged licensed practical nurse Victoria A. Thomas with allegedly being unable to practice with reasonable skill and safety by reason of a mental condition.

During its Board Meeting May 16, the Oregon Board of Nursing revoked the Registered Nurse license of Christopher L. Carlson of Vancouver for failing to cooperate with the Board during an investigation.
