The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.
In July, the Nursing Commission suspended the license of registered nurse Beverly A. Cooke for two years after she tested positive for morphine and admitted to taking patient-returned morphine and oxycodone.
In August, the Department of Health suspended the credentials of registered nursing assistant Rebecca Ann Fay Melcher for two years. Melcher tested positive for marijuana.
In August, the Oregon State Board of Nursing reprimanded Sherrie L. Poujade’s Registered Nurse license for conduct derogatory to the standards of nursing. She resides in Vancouver.
