Health worker report

The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In April, the Mental Health Counselor Program entered an agreement with mental health counselor Sarah Natanya Kohlenberger; she was reprimanded and ordered to undergo two years of monitoring. Kohlenberger didn’t take a patient’s juvenile rheumatoid arthritis into account in counseling the patient.

In May, the Oregon State Board of Nursing reprimanded Evett A. Gradwohl’s registered nurse license for engaging in other unacceptable behavior toward or in the presence of a client. She lives in Vancouver.
