The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In June, the Nursing Commission terminated the order of probation against registered nurse Carolyn Boundey.

In June, the Nursing Commission placed the license of registered nurse Phyllis L. Cimino on probation for two years. She was convicted of offering false instrument for filing or record. She must comply with terms and conditions set against her license.

In July, the Board of Psychology and the Sex Offender Treatment Provider Program released Christopher K. Johnson from terms and conditions set against his license.

In July, the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Denis James Thompson and placed conditions against his credential. Terms and conditions were previously placed against his counselor registration.
