The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In March, the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Sarah Nicole Denis and placed her certification on probation for two years. Terms and conditions were placed against her registration to practice as a counselor. She must comply with terms and conditions set against her certification.

In March, the Board of Pharmacy suspended the pharmacy technician credential of Jessica Duong after the Oregon State Board of Pharmacy suspended her credential to practice in that state.

In March, the Nursing Commission entered into an agreed order with licensed practical nurse Lana M. Leveque. She failed to comply with terms and conditions set against her license. She must comply with new terms and conditions set against her license.

In April, the Nursing Commission charged registered nurse Fairuza M. Stevenson with unprofessional conduct. She allegedly believed medication prescribed by a physician was inappropriate and failed to administer the medication to the patient.
