Health worker report

The Health Professions Quality Assurance office of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In December, the Massage Program granted the application of Loretta Ann Clark and placed her license on probation for five years. Her license to practice as a health care assistant was indefinitely suspended and conditions were placed against her registration to practice as a nursing assistant. She must comply with terms and conditions set against her massage license.

In December, the Dental Commission denied the application of dental assistant David John Agashkov. He was convicted of assault, criminal trespass, minor in possession of alcohol on more than one occasion, disorderly conduct, and robbery.
