The Health Professions Quality Assurance office of the State Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In May, the Dental Commission released dentist Thomas M. Kreger from terms and conditions set against his license.

In May, the Nursing Assistant Program charged registered nursing assistant Katherine M. Jenkins with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her registration.

In May, the Department of Health indefinitely suspended the credential of certified nursing assistant Shalonda D. Hart, who failed to respond to an inquiry about a complaint.

In June, the Nursing Assistant Program and the Board of Pharmacy immediately suspended the registered nursing assistant, pharmacy assistant and pharmacy technician credentials of Larissa M. Michaelis. The Oregon State Board of Pharmacy revoked her credential to practice as a licensed technician in that state.
