Health worker report

The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In August, the Nursing Care Quality Assurance Commission immediately suspended the credential of registered nurse Leona M. Smith pending further disciplinary proceedings. Smith’s credential was revoked by the Oregon State Board of Nursing after she took a photograph of a patient’s bare buttocks without their knowledge or consent and texted the photo to two other people. Smith then tried to intentionally mislead the Board of Nursing by making unfounded accusations against a co-worker.
