Health Worker Report

The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In July, 2014, the Unlicensed Practice Program entered an agreement with Dena Tweten under which Tweten must cease and desist from the practice of dental hygiene and pay a fine of $1,000. Tweten, who has no dental hygienist credential, worked as a dental hygienist.

In November, the Unlicensed Practice Program entered an agreement with Rose Ngo requiring her to cease and desist from practicing massage without a license, and pay a $500 fine. Ngo doesn’t have a license to provide massage, but operates a business advertising massage services.

In April, the Oregon State Board of Nursing accepted the voluntary surrender of Tracy J. Wilkinson’s practical nurse license, for using intoxicants to an extent or in a manner dangerous to herself or others. She lives in Vancouver.
