Health worker report

The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In February, the secretary of health permanently revoked the home care aide credential of Sara Marie Hale. In 2015, Hale pleaded guilty to third-degree assault – domestic violence.

In March, the secretary of health indefinitely suspended the chemical dependency professional credential of Matthew Eastman-Pittam. Eastman-Pittam didn’t comply with requirements to submit a report from a psycho-sexual evaluation and a supervisor’s report.
