Northeast Hazel Dell — The Area Agency on Aging & Disabilities of Southwest Washington’s Primary Care Liaison program earned an Aging Innovations Award from the National Association of Area Agencies on Aging. The liaison program was among the top 16 of 45 local aging programs to receive honors. The program provides targeted outreach and ongoing support to primary care practices and health care providers in the area agency’s five-county service area. The liaison program also educates residents from a family medicine residency program on the aging network and best practices for connecting their patients to community resources.
