The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In October, the Dental Commission ended conditions on the dentist credential and moderate sedation with parenteral agents permit of Phong Thanh Bui.

In November, the secretary of health suspended for at least five years the certified nursing assistant credential of Keri Kaylene Picone. The state Department of Social and Health Services, which found that Picone financially exploited a vulnerable adult, placed her on its long-term care abuse and neglect registry. That prohibits her from being employed in caring for or having unsupervised access to vulnerable adults.

In December, the secretary of health conditionally granted a dental assistant credential to Mindy R. Garcia and ordered her to undergo evaluation for a substance abuse monitoring program. Garcia’s certified nursing assistant and health care assistant credentials were suspended in 2011 in connection with cocaine use.

In December, the Nursing Commission indefinitely suspended the registered nurse credential of Charles Michael Herndon, who didn’t comply with a requirement to enter a substance abuse monitoring program.

In December, the Nursing Commission entered an agreement with licensed practical nurse Wai-Ming Mak that places Mak on probation for at least 12 months. Mak, who must abide by practice restrictions, must complete continuing education in patient advocate and ethics, as well as in patient assessment and document. Mak didn’t perform a full assessment of a post-stroke patient who fell. The patient was subsequently diagnosed with fractures of her left arm.
