Health Worker Report

The Health Systems Quality Assurance Division of the state Department of Health recently took disciplinary actions or has withdrawn charges against these Clark County health care providers.

In July, the Nursing Assistant Program suspended the license of Taylor Allen Altamirano. Altamirano pleaded guilty to third-degree assault and third-degree theft.

In July, the Nursing Commission granted a licensed practical nurse credential to Cory Churner-Tisdale. Churner-Tisdale must comply with terms and conditions including participation in a substance abuse monitoring program.

In July, the secretary of health denied the application of registered nursing assistant Autumn Marie MacGowan. MacGowan received Vicodin and Oxycontin from another staff member at a supported living facility where she worked. She also stated that she was seeking treatment for substance abuse. The Department of Health requested a substance use evaluation within 30 days and she failed to submit it.

In August, the Nursing Assistant Program granted the termination of probation for registered nursing assistant Jessica Ruth Duong.

In August, the Nursing Commission charged registered nurse Dawn M. Frazier with unprofessional conduct. Frazier allegedly admitted to falsifying at least two prescriptions for controlled substances, and filling them for her own use. Frazier failed to comply with a substance abuse referral contract she entered by withdrawing from the substance abuse monitoring program.

In September, the Nursing Commission withdrew charges of unprofessional conduct on registered nurse Walt Tyler for alleged possession of controlled substances or legend drugs without a prescription.

In October, the Massage Therapy Program charged massage therapist Allen W. Holden with unprofessional conduct. Holden allegedly failed to properly document or retain patient records as required.
