The Washington State Department of Health took disciplinary actions or has withdrawn charges against these Clark County providers.

In March 2023 the Dental Commission and the secretary of health ended the conditions on the dental assistant and certified nursing assistant licenses of Mindy R. Garcia.

In March 2023 the secretary of health reinstated the registered nursing assistant license of Lindsay May Walters.

In March 2023 the secretary of health ended the conditions on the medical assistant license of Cinthya Gorostieta Munoz.

In March 2023 the Nursing Commission charged registered nurse Joseph Karl Wiser with unprofessional conduct. In February 2021, Wiser was convicted of driving under the influence. In March 2022, Wiser was convicted of third-degree malicious mischief.

In March 2023 the Massage Therapy Program and Eddie Deal Anzalone agreed to an order that requires he surrender his massage therapy license and that he never resume practice in any capacity. In February 2023, Anzalone pleaded guilty to third-degree rape, indecent liberties without force, and second-degree assault in relation to two patients.

In March 2023 the secretary of health reinstated the certified nursing assistant license of Rachel Marie Teague.

In March 2023 the secretary of health ended probation of the certified nursing assistant license of Jeana Marlene Perkins.

In March 2023 the secretary of health denied the certified home care aide and provisional home care aide licenses of Janet Elizabeth Scurlock. Scurlock did not respond to a September 2022 Department of Health requirement to obtain a substance use evaluation.

In March 2023 the secretary of health permanently revoked the phlebotomist medical assistant license of Brandon Richard Cullen with no right to reapply. Cullen worked without an active credential and he collected a fee from two patients and then failed to deliver their samples to the laboratory.
