The Washington State Department of Health took action in regard to these Clark County health care providers.

  • In April, the Nursing Commission charged licensed practical nurse Michael Ray McCarter with unprofessional conduct. Charges state that in August 2022, McCarter surrendered his license in Oregon after alleged inappropriate behavior toward co-workers.
  • In April, the Dental Commission indefinitely suspended the dental assistant license of Kristina Velasquez Mendoza. In March 2021, Mendoza called prescriptions into a pharmacy for her boyfriend, using two dentists’ names without authorization.
  • In April, the secretary of health and Brittani Ann LaVelle agreed to conditions on her registered medical assistant license. LaVelle must submit progress reports for the next 12 months, and complete four hours of continuing education in the area of theft awareness. In July 2021, LaVelle was convicted of two counts of identity theft in the state of Oregon, a Class C felony.
  • In May, the secretary of health denied the certified nursing assistant credential of Curran E. Fine. In June 2019 in Oregon and in May 2021 in Washington, Fine was convicted of driving under the influence. Fine did not submit a substance-use evaluation report to the Department of Health when requested.
  • In May , the Nursing Commission charged registered nurse Janae Nichole McDaniel, also known as Janae Harper, with unprofessional conduct. Charges state that in July 2021, McDaniel was convicted of conspiracy to commit health care fraud in the state of Montana.
  • In May, the secretary of health ended the probation on the massage therapist license of Erin Kristine Rodgers.
  • In May, the Nursing Assistant Program charged registered nursing assistant Kimberly Ann Smith with unprofessional conduct. Charges state that in 2022, when applying for other medical provider credentials, Smith did not comply with requests for a substance-use evaluation. Smith was convicted of driving under the influence in 2008 and 2020.

The Oregon State Board of Nursing took final disciplinary action against the following Clark County residents at its May 17 board meeting:

  • The board imposed a $7,000 civil penalty on Alma A. Garboden’s registered nurse license for violating a person’s rights of privacy and confidentiality by accessing information without proper authorization or need to know.
  • The board placed the registered nurse license of Robin W. Larson on probation for 24 months for practicing nursing while impaired and using intoxicants to the extent injurious to herself or others.
