State patrol: Inattention thought to have caused fatal Saturday crash

Investigation into rollover that killed three, including 6-year-old, continues

The investigation into a Saturday rollover that killed three, including a 6-year-old girl, after they were ejected from their vehicle was ongoing, the Washington State Patrol said Sunday, but investigators believe inattention caused the crash.

Daryl Tuimavave, 36, of Lakewood was heading north on Interstate 5 about 5 miles north of Woodland around 4:36 p.m. when her Toyota Sequoia went off the right side of the road, according to the state patrol.

The Woodland Police Department said Tuimavave was traveling with her parents and three children, all also from Lakewood, when the SUV rolled multiple times, ejecting the other five occupants.

A 6-year-old girl died at the scene, as did Oferia Nuusola, 60, and Lokeni Nuusolia, 65.

Two other children, a 5-year-old boy and 7-year-old girl, were airlifted to Randall Children’s Hospital. Tuimavave was also hurt, and taken to PeaceHealth St. John’s Medical Center in Longview.

Tuimavave has since been discharged, according to the hospital.

The children’s conditions were not available.

Investigators confirmed Sunday preliminary reports that Tuimavave was the only one in the SUV wearing a seat belt or restraints, the state patrol said, and that some sort of driver distraction caused the rollover.
